Audio degrades past intelligibility well before video starts looking bad, which is why it should be budgeted first rather than treated as the cheap track. In practice that means codec settings chosen for speech rather than for music or bitrate, and measuring intelligibility on the real link instead of inferring it from a number in a config file.

With an append-only audit log that records access as an event rather than a permission, plus a periodic access review. The question a challenge asks is not whether access was restricted but whether you can reconstruct what actually happened — those are different systems, and only the second one survives the question.
